The National Registry (NR) can help you in securing PhilHealth forms and processing your applications for registration and claims. NR members are considered Direct Contributors whether as employees with formal employment, professional practitioners, or self-earning individuals.

 

What is PhilHealth?

 

The Philippine Health Insurance Corporation (PhilHealth) is a government-owned and controlled corporation (GOCC) created through Republic Act No. 7875 or An Act Instituting a National Health Insurance Program for All Filipinos and Establishing the Philippine Health Insurance Corporation for the Purpose.

 

As an attached agency of the Department of Health, PhilHealth aims to provide health insurance coverage and ensure accessible and affordable health care services for all Filipinos.

 

Benefits

 

Inpatient Benefits are paid to accredited Health Care Institutions through All Case Rates. Meanwhile, Outpatient Benefits cover Day Surgeries, Radiotherapy, Hemodialysis, Outpatient Blood Transfusion, and Primary Care Benefit with Expanded Primary Care Benefit.

 

Z Benefits provide comprehensive health care services in the form of Benefit Packages, including but not limited to Acute Lymphocytic / Lymphoblastic Leukemia (standard risk), Breast Cancer, Prostate Cancer, Coronary Artery Bypass Graft Surgery, Cervical Cancer, Colon and Rectum Cancer, Z-MORPH (Mobility, Orthosis, Rehabilitation, Prosthesis Help), and Selected Orthopedic Implants. 

 

There are also Z Benefit Packages for the Prevention of Preterm Delivery, Surgery for Tetralogy of Fallot in Children, and for Children with Developmental Disabilities, Mobility Impairment, Visual Disabilities, or Hearing Impairment.

 

SDG (Sustainable Development Goals) Related Benefits include the Outpatient Malaria Package, Outpatient HIV-AIDS Package, Voluntary Surgical Contraception Procedures, Animal Bite Treatment Package, and Outpatient Anti-Tuberculosis Treatment through Directly-Observed Treatment Short-course (DOTS) Package.

 

How to Register?

 

Registration may be done offline and online. For those who are going to the nearest Local Health Insurance Office (LHIO) or PhilHealth Express branch, submit the following:

 

  • Two (2) copies of the completed and signed PhilHealth Member Registration Form (PMRF)
  • Two (2) pcs of your 1x1 latest ID picture
  • Two (2) valid IDs

 

Await your Member Data Record (MDR) and PhilHealth ID Card. Another option to register is via the PhilHealth Electronic Registration System of the My PhilHealth Portal under PhilHealth’s Online Services.

 

In the Electronic Registration page, accept the Terms and Conditions, enter the required information, and submit your registration. An e-mail will be sent to you containing the transaction number and instructions on how to complete your application. There is also an Electronic Group Enrollment System.
