Fill out the credentials segment of the form. Write your name, email address, and your temporary password. You will receive a password reset link once your application has been approved. Once done, click proceed.

Provide your CORRECT and COMPLETE information.

You must now provide at least three (3) project engagement within the last five (5) years. Input your work engagements by clicking Add Project Engagement.

You may now fill out the fields.

Once done, click Save.

If you have more project  engagements, please feel free to add them as well.

Upload at least one (1) proof of engagement for any of the engagements you listed above. The acceptable proofs are listed as well.

Upload a 1x1 photo of you with a white background, and a copy of one (1) valid government-issued ID. The acceptable IDs are listed for your reference.

Passport, Driver’s Licence, SSS, UMID Card, PRC ID, Voter’s ID, Senior Citizen ID, PWD ID, Philhealth ID, TIN Card, GSIS e-Card, Postal ID, NBI Clearance, Police Clearance, Company ID, Student's ID, Alumni ID, Cedula, PSA Marriage Contract, PSA Birth Certificate, Firearms License Card (With Photo), Credit Card (With Photo)

Once done, click proceed.

Agree to our terms of agreement  by ticking the boxes. Write your full name on the space provided, then upload a copy of your e-signature.

 

You can do this digitally.

 

If unavaible, you can (1) write your signature on a clear, white paper,  then (2) take a photo of your signature. You can now upload this image as your e-signature.

Once everything has been checked, go back to the last page and click PROCEED.

WHAT IS MYFDCPREGISTRY PROFILE?

The MyFDCPRevgistry Profile ia FDCP’s online platform exclusive to registrants of theFDCP National Registry.

It features a customizable profile where registrants can upload their filmography, work engagements, and photographs. Registrats acan also link their IMDB and social media accounts to their respective MyFDCPRegistry profiles.
